COMPLEX NEEDS ASSESSMENT PANEL AND INTERGRATED SERVICES (CNAPIS)
CNAPIS brings government and community representatives in partnership to provide a coordinated approach when assessing and planning the needs of clients with complex needs especially with regards to the complexities inherent of Co-morbidity (AOD and Mental Illness). The CNAPIS function is to address identified issues and barriers by planning, implementing and reviewing the strategies and interventions required to support young lives (17-29) at risk alongside their families/significant other. CNAPIS also functions to advocate for its clients within GCDC services, networks and sectors to ensure the effectiveness of collaborative partnerships.
Who?
Clients are aged between 17 -29 years old, currently receiving income support, who show significant impairment (physical/psychological/personality) in the context of co-morbidity. The client will have had multiple engagement with service delivery with repeated unsuccessful outcomes despite allocation of resources. Client will have multiple external and social issues; impairment with social functioning in need of ongoing support and advocacy to maintain an improved quality of life.
How?
Panel members agree to work using a framework that is consistent and works toward common and agreed goals. CNAPIS identifies and acknowledges the skills and strengths of all stakeholders and the importance of utilizing those strengths to assess and implement case plans. CNAPIS considers client participation as paramount and seeks to develop a case management plan that is set to guide the provision of care; these plans are developed collaboratively and are based on the needs of each individual and the commitment of the Panel members directly involved. CNAPIS members share the responsibility of committing to seeing outcomes for clients and their family/significant other.
Where?
The Complex Needs Assessment Panel and Integrated Services Program is based at 191 West Burleigh Road, Burleigh Heads, Qld, with outreach services at, 9 Scarborough Street, Southport.
Cost?
This program is free of charge to clients.
The Program is funded by the Australian Department of Employment and Workplace Relations.
